Thank you for volunteering with Interfaith Caregivers! Your time, compassion, and dedication make a meaningful difference in the lives of seniors and adults with disabilities throughout Polk County.

To help us continue providing the best possible volunteer experience, we ask all volunteers to complete our annual survey. Your feedback helps us celebrate what's working well and identify opportunities to improve.

Please complete the survey(s) that apply to you:

• Volunteer – Required for all current volunteers.

• New Volunteer – Also complete this survey if you began volunteering in the past year (summer of 2025).

• Regional Rides & General Availability Update – Optional for volunteers who would like to update their driving region or volunteer availability.
